Please be sure to bring the following documents so we can best assist you:
1. Current Photo ID for all adults in the household (driver’s license or state ID); proof of residency for all minors in the household (report card, shot record, tax return, etc.)
2. Proof of homeownership (recorded deed, land contract, probate court order, divorce judgement, etc.)
3. Proof of income: 2020 income for all adults in the home (W2, pension, SSI/SSA/SSD, bridge card, DHS, child support)
